Class 1 and Class

NOTE: The information contained below is a summary of when this form can
be used. The form contains the full information and the information on
the form will take precedence over this summary.

The Class 1 form needs to be updated annually for all participants for
activities not exceeding 72 hours with a level of activity similar to that
of home or school. This form can be completed by the parents.

The Class 2 form is required for Resident (Summer) Camp or any other activity
such as backpacking, tour camping, or recreational sports involving events
lasting longer than 72 hours. A current medical evaluation by a doctor
within the past 36 months is required. This form must be completed by all
participants under 40 years of age.
Class 3 Medical form

The Class 3 form is to be used for all high adventure activities, athletic
competition and jamborees. It is also required for all adults over 40 for
all activities requiring a physical exam.
